Big effect

In a 190-person trial, adding Saccharomyces boulardii to a standard antibiotic regimen pushed H. pylori eradication from 80% to 93% — but the effect comes from a specific rescue therapy in a clinical population, not from general use.

This is the largest effect seen for S. boulardii on H. pylori in the 7-study body of evidence (which on average shows only a small benefit), so while the result is impressive, it may not apply to people on different antibiotic protocols or to asymptomatic carriers.

People who took 250 mg of Saccharomyces boulardii twice daily alongside a 14-day course of vonoprazan and amoxicillin cleared their H. pylori infection more often — 92.6% versus 80% — and reported fewer side effects (11.6% vs. 32.6%). However, this was a rescue regimen for patients who had already failed previous treatment, not a first-line approach, and the study didn't disclose funding sources.

Where this fits in the evidence

Pillser has synthesized 7 studies on saccharomyces boulardii for Improved H Pylori Eradication Rates — overall evidence strength: High.

Across 7 studies, 5 reported beneficial effects of Saccharomyces boulardii on Helicobacter pylori eradication rates, with a predominant small effect size (risk ratios typically 1.09–1.14). Two studies found neutral results. Evidence is strongest for combination with standard triple therapy, with effects seen in both children and adults. Most effective dose reported is 250 mg twice daily for 14 days, though dosing was infrequently reported.
