In clinical studies, peony (as TGP) has been associated with an increased incidence of diarrhea (5.5% vs. 0.9% in one study) and abdominal symptoms (RR=3.27, p<0.001) compared to control. Across 6 studies, two reported statistically significant increases in specific gastrointestinal adverse events, while two other studies found no significant difference in overall adverse reaction rates. Many reports described peony as generally well tolerated with only mild side effects, and one study noted no severe events or discontinuations.

Caveats: Evidence is drawn from short-term studies (mostly ≤8–12 weeks) and many trials were not primarily designed for safety assessment, so rare or long-term adverse events may not be captured. The safety profile in pregnant women, children, or patients with significant comorbidities is not well characterized.
