7 studies reporting safety data2 increased-risk findings1 serious adverse event

In clinical studies of peony extract (total glucosides of paeony, TGP), two specific adverse events were reported at increased rates compared to control: diarrhea (5.5% vs. 0.9% in one study) and abdominal symptoms (RR = 3.27, 95% CI 1.62–6.60, p < 0.001 in another). Two additional studies found no significant difference in overall adverse event incidence between TGP and placebo. Across the remaining studies, TGP was generally described as well tolerated, with mild gastrointestinal events or skin dryness reported and no severe adverse events or treatment discontinuations noted in the available data.

Caveats: Evidence is based on a limited number of clinical trials (7 studies), most of which were short-term and powered for efficacy rather than safety; rare or long-term adverse events may not be captured. The findings reflect the specific TGP formulation used in each trial; other peony extracts or preparations may have different safety profiles.
