Pueraria Mirifica has been associated with increased C-reactive protein (doubled in one study) and a 15% increase in triglyceride levels from baseline (P<0.05) in two clinical studies. Across 4 studies, 10 specific adverse event comparisons showed no significant difference versus placebo, including dizziness, mastodynia, vaginal spotting, endometrial changes, and laboratory test abnormalities. Two studies reported overall tolerability as good, with only mild adverse effects noted. No serious adverse events were reported across the included trials.

Caveats: The evidence is limited to a small number of clinical studies, most of which were powered for efficacy rather than safety, so rare or long-term adverse events may not be detected. Findings reflect the specific formulations and doses tested (25–50 mg) and may not generalize to other preparations or populations, such as those with hormonal conditions or on concurrent medications.
